Measuring Population Health Success at Community Health Centers

How do community health centers know they are making progress in population health? Join the Association, HITEQ Center and Community Health of Central Washington for an engaging session exploring how success is being defined, measured, and used to drive improvement. Through presentations and discussion, participants will gain practical insights into key metrics and tools, discover strategies for analyzing and applying data, and learn from both successes and challenges in evaluating population health programs. 

Attend this learning event to: 

  • Define success in population health programs

  • Describe common metrics and benchmarks used by health centers to measure population health outcomes

  • Identify strategies for applying data to improve care delivery and program impact

Intended audience will include Population Health, Clinical, and Quality Improvement staff from community health centers.

taci has over 13 years of quality improvement and process improvement experience and leads a multidisciplinary team of transition care management nurses and case-managing social workers. Her work centers on improving care coordination, strengthening quality outcomes, and advancing patient-centered care across clinics. Staci collaborates closely with clinical teams and MCO partners to quality improvement initiatives, guides the organization through PCMH recognition and reporting, and supports the development of value-based payment strategies. With a commitment to data-driven improvement and community-focused healthcare, Staci strives to ensure that every patient receives seamless, high-quality care that supports their long-term wellbeing.

Kate has worked in community health for over 14 years. After working in clinic operations for almost a decade, Kate joined the Colorado primary care association to provide quality improvement training and technical assistance to health centers, often leveraging UDS and population health data sources for improvement projects. Since coming to JSI, Kate has served as Project Director for the Bureau of Primary Health Care's Workforce Well-being Technical Assistance Project, the 2024 National Association of Community Health Center Training and Technical Assistance Needs Assessment, and she now leads the HITEQ Center.
