Join WACH for a new Health IT Leadership Series designed specifically for Community Health Center CIOs, IT Directors, informatics leaders, and data teams.
Session Two: October 12, 10:00 - 11:15 AM
The goal of this session is to familiarize community health center IT staff with Community Information Exchange (CIE) capabilities, interoperability goals, and vendor requirements. And to continue to highlight past and future CHC involvement in CIE governance and implementation. This session provides an overview of the WA CIE capabilities, interoperability goals, and vendor participation requirements, helping CHC IT staff understand how WA CIE is designed to connect organizations and support better information exchange across Washington.

Nick Faulkner

Nick Faulkner, MBA, is Vice President of Consulting at Comagine Health and a PMP-certified leader with more than 18 years of experience guiding Medicaid, public health, and health IT transformation initiatives. He has led large-scale modernization efforts across government programs, including Medicaid and public assistance systems, providing expertise in project management, governance, quality assurance, and technology strategy. He has worked extensively with CMS, federal agencies, state legislatures, and other public sector partners to deliver complex technology and program transformation initiatives.
Joanna Den Haan

With expertise in systems design, operations, and community health, Joanna has spent her career improving the ways healthcare and social service organizations collaborate to support population health. Joanna’s experience includes leading quality improvement efforts for chronic disease prevention and management initiatives across Washington, Oregon, and New Mexico, as well as operations leadership within a large FQHC in Seattle. She also brings more than a decade of experience designing innovative healthcare and social care programs for people experiencing homelessness.
Theresa Kreiser

Theresa's work focuses on sustainable implementation of evidence-based programs and building community-clinical linkages, including addressing health-related social needs in community-based settings. Her role as Training and Technical Assistance Coordinator for the WA CIE focuses on workflows and incorporating user feedback for successful implementation. Theresa holds a master’s degree in Exercise Science: Physical Activity and Public Health from Northeastern University. She was a CDC Lifestyle Coach in Diabetes Prevention for a Virtual Diabetes Prevention Program and is a current Healthy Heart Ambassador Master Trainer.
